Nominations are open to everyone, and self-nominations are welcome.
Use the form below to nominate yourself, another person, or an organization for an Award for Impact.
Be prepared to briefly describe who/what you are nominating and why it deserves recognition in the selected award category.
Nomination write-ups are limited to 250 words.
